Your First Dental Visit
When you call us to make your appointment, you’ll hear a friendly voice on the phone, and you’ll be greeted warmly when you come. We’ll call you back, spend a few minutes chatting with you, and clean your teeth thoroughly, removing calcified plaque. Then, we’ll examine your mouth to make sure that there are no signs of trouble, and we’ll talk about your concerns and goals. If necessary, we’ll also formulate a treatment plan and schedule future appointments.
New Patient Forms
If you are a new patient to our practice, we’ll need you to fill out some paperwork. These forms include your insurance information, your medical and dental history, and more. Please take a little bit of time to complete these forms before you arrive so that we can stay on schedule with your appointment.
